затяжная диарея
сегодняшний вызов.
М 25. Болен 2 недели. ранее ничем подобным не страдал. Не пьет. Больше в семье (еще 3 человека) никто не заболел. жалобы - водянистый стул, без примесей крови. последнее время - после еды. слабость. последние 2 дня температура к вечеру до 39. При осмотре: Состояние отн удовл. Кожа чистая. обычн влажности, обычн цвета. склеры норм. В легких везикулярно с жестким оттенком. ЧД16. хрипов НЕТ. ЧСС 68. АД 100/60 (со слов мамы - по проф. медсестры - у него всегда такое, и вобще у них вся семья гипотоников). отеков нет. живот умеренно болезненный особенно над сигмовидной, чуть меньше - над слепой. урчит и булькает. перитонеальных нет. успел обследоваться. в ОАК лейкоциты 9,7 (формула норм, без сдвигов). СОЭ 45. моча норм. - оч. много фосфатов кажется. флюорография - "признаки инфильтрации в ср доле справа, при наличии клиники вести как пневмонию" (клиники нет). УЗИ - умеренное увеличение печени. сдали баканализ в четверг будет готов. Принимали сульгин, кажется фуразолидон, мезим. От гоститализации в инфекцию с ПТИ отказались. Самостоятельно решили колоть цефтриаксон "от пневмонии". Что это такое? Затянувшееся ПТИ? Что-то другое? Какие еще данные/обследования нужны? ох... |

А где же общий анализ кала (например, лейкоциты - да, нет?), это важно для диф. диагноза и традиционный посев кала (в работе)? Флюорография цифровая? Надо бы вывесить ее здесь.
P.S. С точки зрения диарейного синдрома, это острая диарея (менее 3-х недель). А гипотонии и нет, зря они себя гипотониками называют |

о.а.кала нет пока. сделали баканализ (?) он в работе. посев тоже не делался. Фля - цифровая, если смогу - вывешу.
Дифдиагностику с ПТИ проводить в каких направлениях? Ох... мне завтра с инфекционистом общаться.... |

У больного имеется водянистый стул без примесей, крови, болей в животе. Т.е., невоспалительный диарейный синдром. Что еще можно сказать без анализов? Общего анализа кала нет, ОАК с формулой тоже нет. Пока очевидно, что показана оральная регидратация и симптоматическое лечение (парацетамол при выраженной лихорадке) . По поводу пневмонии - нужно уточнять. Особенно если учесть, что понос - 2 недели, а лихорадка присоединилась только сейчас... Молодой мужик, понос, инфильтрация в легком без четкой клиники...если все это дело не разрешится прозаично, в уме СПИД держать будет нелишним

анализ на ВИЧ отрицательный, но не будем терять надежды
ДРУГ ...ну может не совсем точно процитировал, но смысл именно таков. в последнее время очень много таких заключений ФлГ, после того случая с пропущенной на догоспитальном этапе пневмонией с несчастливым концом. |

Это ценная информация. Нужно идти к рентгенологам и смотреть снимок на их экране (и на флешку копировать, чтобы вывесить сюда), переснимать с боковой проекцией. Всякое бывает. У нас обследовался пациент с лихорадкой неясного генеза, на прямом ренгене ничего особенного не было. Абсцесс легкого выявили только тогда, когда он дренировался

Ну что, никаких новостей? Тогда пока выложу чтиво (иначе говоря, почему я так настаивал на копрограмме). В данном случае ее отсутствие и отсуствие ОАК с формулой очень плохо. В итоге - никому ничего не ясно, есть ли пневмония и что на рентгене - непонятно.
DIARRHEA, ACUTE ▲ Ask the following questions: Is there blood in the stool? From the algorithm, blood in the stool should indicate that there is Salmonella , Shigella , Campylobacter jejuni , ulcerative colitis, and amebic dysentery. Without blood in the stool, it is more likely that the acute diarrhea is due to a staphylococcal toxin, giardiasis, traveler's diarrhea, a virus, or contaminated food. Is there a fever? Fever, especially with an elevated white count and blood in the stool, would suggest Salmonella , Shigella , Campylobacter jejuni , or ulcerative colitis in its acute stage. The absence of fever would suggest amebic dysentery or giardiasis, although there may be fever in amebic dysentery in the severe cases. Even traveler's diarrhea and toxic staphylococcal gastroenteritis do not usually give more than a low-grade temperature at best. Pseudomembranous colitis may result in a significant elevation of the temperature once the patient becomes severely dehydrated. Is there severe vomiting? Severe vomiting is seen in toxic staphylococcal gastroenteritis! This follows 2 to 4 hr after eating food poisoned with the toxin. Traveler's diarrhea and viral gastroenteritis may also cause severe vomiting, as may food that is contaminated. On the other hand, there is little or no vomiting in giardiasis and pseudomembranous colitis. Did several members of the family experience acute diarrhea also? This is a key question because it indicates whether there is a possibility of toxic staphylococcal gastroenteritis or the possibility of a contagious condition such as infection with Salmonella , Shigella , or Campylobacter . If only one member of the family was suffering from diarrhea and everyone is eating the same food, then it is less likely to be a contagious condition, and one must consider ulcerative colitis, pseudomembranous colitis, and conditions listed under chronic diarrhea. Was there recent foreign travel? Recent foreign travel would suggest the possibility of traveler's diarrhea, cholera, shigellosis, salmonellosis, and giardiasis. Is there neurologic symptomatology? This should point one in the direction of botulism, and generally a little epidemiologic research will disclose that other people in the community have been suffering from the same condition. ▲TopDIAGNOSTIC WORKUP The first thing to do is a stool for occult blood. This will help distinguish those patients who are having obvious infectious disease of the large intestine or maybe even the small intestine. It will also make one suspicious of ulcerative colitis. All patients need a stool culture and stool for ova and parasites. A stool for Giardia antigen can also be done. Serologic studies will not be of much help in the acute condition, but they may help later on in cases of salmonellosis and amebiasis. The clinician himself should do a methylene blue smear for leukocytes and examine a wet saline preparation of the stool. If there is a history of antibiotic uses, a stool should be tested for Clostridium difficile toxin B. Leukocytes on a smear suggest bacterial cause and a culture should be done. The laboratory should be alerted if Campylobacter or Yersinia are suspected because special culture media are needed. If the diarrhea persists or if there is blood, sigmoidoscopy or colonoscopy should be performed. It is always important to examine the rectum for hemorrhoids and anal fissures that may be causing the positive stool for occult blood. When the diarrhea persists and becomes chronic, the diagnostic workup should include the studies that are listed under chronic diarrhea. Анализ кала на патогенную флору, а так же на яйца глист и прочих ничего патогенного не выявил. Пациент госпитализирован в пятницу в многопрофильный стационар с о взятым с неба диагнозом. Со слов родственницы, первые сутки провел в диагностическом отделении. Переведен в терапию по поводу рентгенологически подтвержденной пневмонии. Сохраняется высокая температура >38, то возникают, то исчезают боли в животе. Якобы его регулярно смотят хирурги по поводу ? диагноза. Завтра придут лечащий врач и ко - будут новости.
зы: В ОАК - только высокая СОЭ. формула нормальная. Лейкоциты в норме. были по крайней мере до госпитализации. |

Аnton! Не исключаю, что у пациента так называемая атипичная пневмония, при которой встречаются внeлёгочные симптомы. Ето достаточно распространённые заболевания, отвечaюwие правильному лечению.

Про микоплазму я поначалу подумал, но к ней желательно иметь кашель . Хотя такая штука - именно из-за этих чудес с атипичными пневмониями предпочитаю начинать с макролидов
