Сама хотела сказать про это, но еще вот есть какие мысли: актилизе как-то была на вооружении областной больницы. Вместе со стрептокиназой использовалась для тромболизиса при ОИМ. Об инсультах никто и не заикнулся. И еще: сейчас идет закупка отечестенного оборудования в города и веси, иногда (по собственному и чужому опыту) совершенно бессмысленная - огромные деньги на ветер. У нас больных сахарным диабетом теперб после лечения (иногда не показанного) в стационаре еще и в санаторий отправляют, на долечивание. Что они там делают - загадка. А денег на одного больного - примерно как на актилизе. Между тем, если больной поступает в кетоацидотической коме, у нас нет возможности определить калий и pH Деньги-то тратятся, только не на то, что нужно.

Вступлю в дискуссию как человек, который может и должен...
Для начала, для тех, кто еще не знает Я работаю врачом в ОРИТ. Однако, de-facto, наш ОРИТ является кардиобитом. Т.е., в асболютном большинстве случаев мы лечим (и, говорят, хорошо лечим!) острые инфаркты, а также жизненно опасные нарушения ритма и проводимости, тяжелую сердечную недостаточность и т.п. В то же время, кроме основной так сказать специализации, мы работаем еще и как общая терапевтическая реанимация, "прикрывая" несколько клиник Клинического Центра ММА (для внутригоспитальных острых ситуаций). Таким образом, в случае развития внутригоспитального инсульта (>90% - ишемические), эти больные попадают к нам. Техническое обеспечение следующее: КТ и МРТ только по будням с 9-00 до 18-00 (ох, сколько времени мы пытаемся убедить начальство, что отсутсвие экстренного КТ в клиническом центре - полное и абсолютное безобразие...). Альтеплазы - залейся!!! (особенности снабжения крупного государственного медицинского учреждения федерального подчинения ). Дежурный невролог доступен, как правило, в течение 1 часа (хотя это, строго говоря, формальность, диагноз ИИ мы и сами спосбны поставить). Ну так вот, к чему это я? А к тому, что при всем нашем желании и возможностях, мы так и не сделали ни одного тромболизиса при доказанном ИИ менее 3 часов. КТ есть только днем, а в это время кругом много осмотрительного опасливого начальства . Так вот и живем... Впрочем, я осознаю, что главное - начать. Пару раз сделаешь тромболизис, а там уж это перестанет быть "новостью" и станет применяться само собой . |

Позволю себе встрять. 8 лет отработал в нейрососудистой реанимации в ГКБ№6. Пытались проводить тромболизис при ИИ в острейшем периоде (1995-1997гг.). При обширных инфарктах мозга несколько раз получили геморрагическое пропитывание ишемического очага.
Соглашусь, что принципы ЕВМ в пользу лизиса. Но попасть на "красный" тромб, при котором лизис эффективен - тоже лотерея! Поэтому, на мой взгляд, для того, чтобы лизировать ИИ, стоит учитывать две вещи: доказанность тромбоэмболического характера ИИ и отсутствие окклюзирующего процесса в МАГ, ИМХО. |

Помнится, один раз у нас с Аней все совпало. И КТ сделали и альтеплаза была... Увы, инсульт оказался геморрагический. Так что сделать тромболизис не удалось. Еще несколько раз вводили альтеплазу (внутриартериально) люди, которые у нас занимаются стентированием сонных. А так, как справедливо сказала Аня, мы бы и рады, но не можем.

Реальный инсульт поступает в лечебницу через 6, а то и 12 часов. Так что речь только о внутрибольничных. Так и с ними геморрой, даже и днем. Пока с КТ договоришься, пока то, пока се - "золотой час" тю-тю. Да и определенная решимость администрации нужна. И еще. Мне кажется, что гораздо большую пользу в нашей стране принесет не тромболизис, а внедрение нормальных реабилитационных программ (это не в порядке противопоставления). Они и эффективны и безопасны. Но гораздо белее трудоемки. И еще создание специализированных инсультных БИТов. А то их в Москве-то всего - ничего.

Background and Purpose--The most common presentation of ischemic stroke related to pregnancy is arterial occlusion, occurring during the third trimester or postpartum. The authors present the first successful administration of intra-arterial cerebral tissue plasminogen activator to treat an embolic cerebral vascular accident in a 37-week parturient resulting in complete recovery of neurological function. Methods--The patient presented with left hemiplegia, left-sided neglect, and aphasia. Right internal carotid artery cerebral angiogram showed occlusion of the mid-M1 segment of the middle cerebral artery (MCA). After 15 mg of tissue plasminogen activator was administered via intra-arterial route, there was greatly improved retrograde flow through the posterior communication artery and the MCA territory. Results--A reduction in size of the MCA occlusion was noted with improvement of antegrade flow through the MCA. Three days after the procedure, the patient was induced successfully and delivered a healthy infant vaginally. Conclusion--This report describes the use of intra-arterial tissue plasminogen activator in the setting of stroke in late pregnancy. Удивлен что пропустил эту тему. Хотя создана она немного не в том разделе. По сути это больше проблема неврологии, чем АИР. Даже более - проблема не интенсивки, а организации. Мы занимаемся перманетно этим вопросом с 2002 года. С мая 2006 официально для проведения тромболизиса готово все что можно, и не только в одной клинике. Однако на настоящий момент нехватает главного - пациентов! Все ждем. По сути всякрывается глобальнейший пласт проблем организации неврологической помощи пациенту на догоспитальном этапе. Все начинается с необразованности пациентов, недоверия к отечественной медицине. Ведь чтобы все подпадало в сроки - необходимо доставить больного в стационар в течение 2-х часов от начала инсульта. А для этого больному надо сделать вызов в течение примерно 1 ч 20 мин после появления первых симптомов. Конечно такие больные есть, но как назло у них в наличии противопоказания. Т.е. чтобы как-то система заработала, прежде всего надо начинать с мощнейшей пропаганды из СМИ и прочих обучающих программ для пациентов. А с прискорбием можно сказать, что на это денег надо гораздо больше, чем на приобретерие препаратов и оснащения. ИМХО на сегодняшний день церебральный тромболизис остается новой технологией, модной фишкой. Реальных перспектив его развития в России пока не видно. |

Интра-артериальное введение урокиназы улучшает прогноз при кардиоэмболическом инсульте и может стать альтернативой тромболизису тканевым активатором плазминогена. Д-р Takeshi Inoue и его коллеги (Национальный Сердечно-сосудистый Центр, Осака) поясняют, что в Японии применение тканевого активатора плазминогена (ТАП) для тромболизиса еще не получило официальный статус. Поэтому при острой окклюзии церебральных сосудов часто применяется интра-артериальное введение урокиназы (УК). Преимуществами последней, по сравнению с ТАП, может быть и большее терапевтическое окно (6 ч против 3 ч). В рамках исследования J-MUSIC (Japan Multicenter Stroke Investigators Collaboration) авторы сравнили данные 91 пациента с кардиоэмболическим инсультом, поступавшего в стационар в первые 4.5 ч и получавшего УК интра-артериально, и показатели группы контроля (182 больных с инсультом, не получавших УК). Оказалось, что благоприятный клинический исход, оцениваемый по модифицированной шкале Rankin при выписке, достоверно чаще наблюдался в группе УК: 50.5% против 34.1% в группе контроля. Различия в уровне смертности (11.0% и 13.3%, соответственно) не достигали статистической значимости. По продолжительности пребывания в стационаре обе группы не различались. "Таким образом, тромболизис с помощью интра-артериального введения УК эффективен у больных с острым ишемическим инсультом и позволяет расширить терапевтическое окно… Это позволяет выполнять тромболизис у большего числа пациентов", заключают исследователи ели в новом номере Cerebrovascular Diseases. Cerebrovasc Dis 2005;19:225-8. Комбинированный, внутривенный и интраартериальный, тромболизис существенно эффективнее, чем изолированный, у больных с ишемическим инсультом. Д-р M Flaherty и его коллеги (Медицинский Центр Университета Цинцинати, Огайо) объясняют, что преимущество комбинированного, внутривенного (ВВ) и интраартериального (ИА), тромболизиса - возможность ранней доставки рекомбинантного тканевого активатора плазминогена (ртАП) непосредственно к тромбу. Авторы обследовали 62 больных с острым ишемическим инсультом, отвечавших стандартным показаниям к тромболизису. Вначале пациенты получали ВВ инфузию ртАП (0.6 мг/кг, в течение 30 минут), затем выполнялась ангиография церебральных сосудов и ИА введение 22 мг ртАП, в течение 2 ч. Лечение начиналось в среднем через 129 минут после появления первых симптомов. Через 3 месяца функциональный клинический исход был благоприятен у 50% больных (баллы 0-2 по модифицированной шкале Rankin). Общая смертность была достаточно низкой (18%), симптомное кровотечение из церебральных сосудов отмечалось у 8% пациентов. Однако среди лиц старше 80 лет эти показатели увеличивались до 63% и 25%, соответственно. В этой возрастной группе был хуже и функциональный статус участников. Methods--Eleven consecutive patients with stroke attributable to acute occlusion of a major cerebral artery were treated with an intravenous bolus injection of the platelet glycoprotein IIb/IIIa antagonist tirofiban combined with heparin and by endovascular procedures including mechanical thrombolysis and locally delivered urokinase. Of the 11 cases, 9 involved angioplasty and 2 only microcatheter and microguidewire manipulation. Results--There were 7 patients with internal carotid or middle cerebral artery occlusion treated within 6 hours and 4 patients with basilar artery occlusion treated within 12 hours of symptom onset. Median National Institutes of Health Stroke Scale (NIHSS) score on admission was 20. After the interventional procedure, vessel recanalization was partial (thrombolysis in myocardial infarction grade flow 2 [TIMI 2]) in 7 patients and absent or insufficient in 4 patients. Twenty-four hours after the procedure, all the patients but 1 improved substantially, and on control angiography, the occluded vessel was totally patent (TIMI 3) in 10 of the 11 patients. One patient with partial recanalization did not improve and died 3 months later from pulmonary embolism. Neither a symptomatic intracerebral hemorrhage nor systemic bleedings requiring blood transfusion occurred in any patient. At discharge, median NIHSS score was 2. The 3-month outcome was excellent in 8 patients (modified Rankin Scale [mRS] 0 to 1), good in 2 patients (mRS 2), and poor in 1 patient (mRS 6). Conclusions--The combination of intravenous tirofiban with intra-arterial mechanical clot disruption and urokinase may be successful in reopening an occluded major cerebral vessel without increasing the hemorrhagic risk and with good functional outcome. This strategy cannot be recommended as the systematic treatment of stroke attributable to major cerebral artery occlusion until tested in a controlled study design. |

А вот вопрос интраартериального (селективного) тромболизиса как раз более детальный и оснащенческий, чем глобально организационный.
Насколько помню интраартериально урокиназу начали вводить уже давно. Тут есть важное отличие: должен быть специально проученный человек с опытом. Наверное в России его получить сложновато. В одной фразе в чем будет заключаться методика: введение микрокатетером препарата под контролем ангиографии до тромба, и после тромба, протыкая его. Получается и химическое разрушение и, отчасти, механическое. Ежели неселективный птромболизис может проводить широкий круг врачей, то таким ювелирным действам надо еще обучиться. |

При ОИМ интраартериальный тромболизис не прижился. Посмотрим, как будет в этом случае. Пока, на сей момент, общепризнанной считается методика системного тромболизиза только преператом t-Pa. Считается перспективным применение десмотеплазы, но это еще на стадии изучения.

Methods— DEDAS was a placebo-controlled, double-blind, randomized, dose-escalation study investigating doses of 90 µg/kg and 125 µg/kg desmoteplase. Eligibility criteria included baseline National Institute of Health Stroke Scale (NIHSS) scores of 4 to 20 and MRI evidence of perfusion/diffusion mismatch. The safety end point was the rate of symptomatic intracranial hemorrhage. Primary efficacy co-end points were MRI reperfusion 4 to 8 hours after treatment and good clinical outcome at 90 days. The primary analyses were intent-to-treat. Before unblinding, a target population, excluding patients violating specific MRI criteria, was defined. Results— Thirty-seven patients were randomized and received treatment (intent-to-treat; placebo: n=8; 90 µg/kg: n=14; 125 µg/kg: n=15). No symptomatic intracranial hemorrhage occurred. Reperfusion was achieved in 37.5% (95% CI [8.5; 75.5]) of placebo patients, 18.2% (2.3; 51.8) of patients treated with 90 µg/kg desmoteplase, and 53.3% (26.6; 78.7) of patients treated with 125 µg/kg desmoteplase. Good clinical outcome at 90 days occurred in 25.0% (3.2; 65.1) treated with placebo, 28.6% (8.4; 58.1) treated with 90 µg/kg desmoteplase and 60.0% (32.3; 83.7) treated with 125 µg/kg desmoteplase. In the target population (n=25), the difference compared with placebo increased and was statistically significant for good clinical outcome with 125 µg/kg desmoteplase (P=0.022). Exclusion Criteria: Females of childbearing potential, persons who weigh < 50 kg (110 lbs), who are ineligible to receive medication within 30 minutes of their MRI, whose strokes are idiopathic, who have a prestroke modified Rankin score (MRS) > 1, who have a history of intracranial hemorrhage (ICH), subarachnoid hemorrhage (SAH), arteriovenous malformation, or aneurysm, whose clinical presentation suggests SAH, who use oral anticoagulants or have an international normalized ratio (INR) > 1.7, who have received heparin within 48 hours or who have a partial thromboplastin time (PTT) > 1.5 times the normal value, who use glycoprotein Iib/IIIa inhibitors, or who have any condition that would confer additional risk while receiving trial medication, will be excluded from the study. Patient Involvement: Upon admission, prospective participants will undergo a complete history and physical, an MRI, laboratory tests, and other trial-related assessments in order to determine trial eligibility. Desmoteplase will be administered to participants in a single-bolus IV infusion. Patients will undergo 11 assessments during the 3-month trial, in addition to another assessment upon discharge from the hospital. Primary Outcome: The 90-day clinical outcome, determined by NIHSS, MRS, Barthel Index (BI) scores, and mortality rates, as well as the 30-day biological outcome, measured in areas of PWI and DWI deficits on MRI, PWI/DWI mismatch, change in reperfusion of the affected artery at 4-8 h from baseline (assessed by MRA, magnetic resonance angiography), and incidence of symptomatic ICH.
