Äàâíî áû è ñ óäîâîëñòâèåì ñäåëàë ýòî, íî êâîòû äëÿ êàðäèîõèðóðãèè ó íàñ òîëüêî â Íîâîñèáèðñê (áîëåå 1000 êì îò íàñ). Áîþñü, îí íå òðàíñïîðòàáèëüíûé, äà è íå îïåðàáèëüíûé ïî îáùåìó ñîñòîÿíèþ. Èíòåðåñíî, çàâÿçàâøèé íàðêîìàí ñ ãåïàòèòàìè  è Ñ, äâóñòîðîííåé äåñòðóêòèâíîé ïíåâìîíèåé è òîëüêî ÷òî ðàçðåøèâøåéñÿ ÎÏÍ âîîáùå ñîîòâåòñòâóåò êðèòåðèÿì âêëþ÷åíèÿ äëÿ êàðäèîõèðóðãè÷åñêîé ïîìîùè?

Âñ¸ òå÷åò, âñ¸ ìåíÿåòñÿ. Ïðîøëî 6 ëåò ñ íà÷àëà îïðîñà - íå çíàþ, ãäå êàê, à â ìîèõ ïåíàòàõ êàðòèíà ñóùåñòâåííî ïîìåíÿëàñü.
 2009 ãîäó è ïðåäñòàâèòü ñåáå íå ìîã, ÷òî îñíîâíîé ãîëîâíîé áîëüþ ïðè ëå÷åíèè èíôåêöèé ìîæåò ñòàòü áàíàëüíàÿ K.pneumoniae, à âîò ïîäè æ òû! Ïðèøëà áåäà, îòêóäà íå æäàëè. Ó æå íåîäíîêðàòíî ó ðàçíûõ ïàöèåíòîâ ñ ðàçíûõ îòäåëåíèé è èç ðàçíûõ ëîêàëèçàöèé âûñåâàåòñÿ êàðáàïåíåì-ðåçèñòåíòíàÿ êëåáñèåëëà. À áûâàåò òàê, ÷òî è íå òîëüêî êàðáàïåíåì-, íî è ïàíðåçèñòåíòíàÿ. È åñëè ñ ïàíðåçèñòåíòíîé ñèíåãíîéêîé åùå êàê-òî ñïðàâëÿëèñü ñàíèòàðíî-ãèãèåíè÷åñêèìè ìåðîïðèÿòèÿìè, óâåëè÷åíèåì äîçèðîâîê, ïðèìåíåíèåì ïðåïàðàòîâ off-label, òî ñ êëåáñèåëëîé òàêèå øòóêè ïîêà íåïðîõîäÿò. Îíà ðåàëüíî óñòîé÷èâàÿ. Ïðàâäà, ÷óòü îáíàäåæèë ñëåäóþùèé êëèíè÷åñêèé ñëó÷àé: Ïîñòîÿííàÿ ïàöèåíòêà îíêîëîãè÷åñêîãî îòäåëåíèÿ, ïåðåíåñøàÿ íåñêîëüêî êóðñîâ ÏÕÒ, èñòîùåííàÿ, ãîñïèòàëèçèðîâàíà ñ îáîñòðåíèåì õðîíè÷åñêîãî êàëüêóëåçíîãî ïèåëîíåôðèòà. Èç ìî÷è âûñåÿíà K.pneumoniae, óñòîé÷èâàÿ êî âñåì ïðîòåñòèðîâàííûì àíòèáèîòèêàì, âêëþ÷àÿ êàðáàïåíåìû, êîëèñòèí, òàéãåöèêëèí. Øòàìì áûë ïåðåäàí â äâå êîíòðîëüíûå áàêëàáîðàòîðèè - ËÎÊÁ è ÍÈÈÄÈ.  íèõ, íåçàâèñèìî äðóã îò äðóãà, ïîäòâåðæäåíà ïîëèðåçèñòåíòíîñòü è îáíàðóæåíà ÷óâñòâèòåëüíîñòü ê 2-ì ïðåïàðàòàì: õëîðàìôåíèêîëó è êî-òðèìîêñàçîëó (ïðè óñòîé÷èâîñòè ê êîëèñòèíó!). Õëîàìôåíèêîëà èíúåêöèîííîãî óæå äàâíî íå âèäåëè, äà è íå ïðèìåíÿåòñÿ îí ïðè óðîèíôåêöèÿõ, à ñ áèñåïòîëîì âñ¸ âûøëî ïî÷òè ñìåøíî: ïàöèåíòêà, ïîòðàòèâøàÿ íå îäèí äåñÿòîê (à òî è ñîòåí) òûñÿ÷ ðóáëåé íà ñâî¸ ëå÷åíèå, óñëûøàâ, ÷òî åé ïðåäëàãàþò áèñåïòîë, ïîäóìàëà, ÷òî âðà÷è ðåøèëè îò íå¸ îòìàõíóòüñÿ è ñ÷èòàþò åå áåñïåðñïåêòèâíîé.  îáùåì, äâà äíÿ îíà ïîëó÷àëà áèñåïòîë, òåìïåðàòóðà ñíèçèëàñü, îáùåå ñàìî÷óâñòâèå óëó÷øèëîñü, è îáèæåííàÿ íà ðîññèéñêèõ âðà÷åé ïàöèåíòêà óåõàëà ïðîäîëæàòü ëå÷åíèå â Ãåðìàíèè. ×åì äåëî çàêîí÷èëîñü, óâû, íå çíàþ. Íî, ïîõîæå, "èíîãäà îíè âîçâðàùàþòñÿ" (Ñ). Ýòî ÿ ïðî àíòèáèîòèêè. À ãîëîñîâàëêó íàäî äåëàòü íîâóþ. |

íå çíàþ ïðî àìåðèêàíñêèå, íî ôðàíöóçû âîò ÷òî ïèøóò î ñâîèõ:
All isolates were resistant to all penicillins and expanded-spectrum cephalosporins, and they exhibited heterogeneous decreased susceptibilities to carbapenems, with ertapenem resistance, imipenem being in the intermediate or susceptible range, and meropenem and doripenem being in the susceptible range according to Clinical and Laboratory Standards Institute guidelines updated in June 2010 (10). Èíäóñû ïîêàçàëè âíå çàâèñèìîñòè îò ãåíîòèïà ÷òî ñðåäè 17 êëåáñèåëë, ïðîäóöèðóþùèõ êàðáàïåíåìàçó, òîëüêî ó äâóõ îáíàðóæèëàñü ÷óâñòâèòåëüíîñòü ê áèñåïòîëó (12%), ~10% ÷óâñòâèòåëüíîñòü òàêèõ êëåáñèåëë ê áèñåïòîëó òîæå îòìå÷àåòñÿ è äð. àâòîðàìè:
In 17 KPC strains, it was found that colistin and polymyxin were found to be 100% sensitive followed by imipenem (47.06%), tetracycline (35.29%), cotrimoxazole (11.76%) and amikacin (11.76%)... Similar to our study, A Zogorianou et al reported resistance to Amikacin 74%, ciprofloxacilin 98%, co-trimoxazole 91% in KPC producing Klebsiella pneumoniae. Ò.å., ïî êðàéíåé ìåðå ÿñíî, ÷òî ÷óâñòâèòåëüíîñòü ê êî-òðèìîêñàçîëó, àìèêàöèíó è òåòðàöèêëèíàì (òàéãåöèêëèíó) íå èìååò ÷åòêîé êîððåëÿöèè ñ íàëè÷èåì/îòñóòñòâèåì êàðáàïåíåìàç. Óæå õîðîøî.
