Скрининг рака при прогрессирующей венозной тромбоэмболии
Здравствуйте, коллеги-онкологи. Работаю в кардиохирургической клинике, которая, в числе прочего, занимается тромбоэмболиями легочной артерии. Периодически приходится сталкиваться со случаями распространенного ТГВ и массивной ТЭЛА у молодых людей, не имеющих очевидных (или имеющих минимальные) факторов риска подобных состояний. Понятно, что показан весь (доступный) скрининг тромбофилии. Но памятен случай 26 летнего спортсмена, у которого менее чем за месяц затромбировались на фоне максимального лечения и после тромболизиса все вены нижней половины тела, что и явилось непосредственной причиной смерти. Основным заболеванием оказалась низкодифференцированная зл.опухоль, исходящая из ЖКТ (предположительно, поджелудочной железы), что удалось установить по биопсии метастаза в печени и целенаправленному исследованию онкомаркеров. Первичной опухоли никакими методами обнаружить не удалось.
Вопрос: не встречались ли вам, уважаемые коллеги, протоколы "поиска" опухоли, когда ее не видно, но очень сильно кажется, что она есть? Мы этих больных вынуждены выписывать по месту жительства, но очень хочется дать им план дальнейшего поиска; выписывать с мыслью "вы умрете, мы не знаем от чего, но это уже и не важно", согласитесь, ужасно. Понятно, что в такой протокол входят рентген ОГК, общеклинические анализы (после лечения по поводу ТЭЛА, естественно, уже сделаны), гинеколог/уролог, ФГДС, от себя добавляем КТ ОКГ и ОБП (тут хотелось бы заручиться какими-нибудь рекомендациями). А еще? Есть ли какой-то перечень исследований для разных полов и возрастов? Заранее спасибо. |

Спасибо koriatus, похоже, представленная схема для нашего случая подходит. Мы не так уж много и забыли, как оказывается.

Given the association between idiopathic VTE and occult cancer, it has been suggested that patients with unprovoked thrombosis routinely undergo investigation for underlying malignancy. However, there is little evidence to date that routine cancer screening would be worthwhile or cost-effective in this situation. The preliminary results of a small randomized trial evaluating extensive screening versus no screening in patients presenting with idiopathic VTE were reported recently.50 The battery of tests used for extensive screening included ultrasound and computed tomography of the abdomen and pelvis, stool guaiac examination, gastroscopy, colonoscopy, sputum cytology, mammography, manual pelvic or prostate examination, and measurement of tumor markers (eg, prostate-specific antigen and carcinoembryonic antigen). Thirteen of 99 patients allocated to the extensive screening group compared with 0 of 102 patients in the control group were initially found to have cancer by these means. During the 2-year follow-up period, a diagnosis of cancer was established in 10 patients in the control group and 1 in the screened group. There was no significant difference in cancer-related mortality between the 2 groups (3.9% versus 2.0%, respectively), although this finding may be a reflection of the small number of study subjects. Still, these results give rise to the preliminary conclusion that earlier diagnosis of cancer does not translate into improved prognosis and survival. In patients with idiopathic VTE, supplementation of a comprehensive medical history and a physical examination with basic blood work, and a chest x-ray in smokers, can be expected to detect ≈90% of occult cancers.51

Спасибо, Вадим Валерьевич, сама я почему-то эту статью не нашла. То, что нахождение причины почти точно не повлияет на прогноз, понятно. Но пациентам (а это категория людей, еще недавно считавших себя абсолютно здоровыми) необходимы подтверждения того, что они тяжело больны.. Даже если это не cost-effective.

Вчера не было много времени и привел первый попавшийся пример (тезисы оригинала которого [Ссылки доступны только зарегистрированным пользователям ] ), + недавние публикации говорят о следующем -
норвежцы говорят, что ограниченный скрининг может быть полезен для выявления опухоли после тромбоза, но он положителен и у пациентов без опухоли (коих в 2 раза больше) - всего 10% частота выявления опухоли в течение года: Of 974 patients with venous thromboembolism, 499 had idiopathic venous thromboembolism. Malignancy was detected in 47 of these (9.4%; 95% CI 7.1-12.3) during the first year after the thrombosis. Limited screening detected 44 of these 47 cases (94%) and had a negative predictive value of 99%. [Ссылки доступны только зарегистрированным пользователям ] И напоследок, работа из Чехии о той молодой популяции, что Вы спрашивали: менее 50 лет (средний возраст 35 лет) - частота выявления опухоли всего 2% за 29 мес или 1.2% в год, похоже достаточно усеченного скрининга, чтобы обнаружить опухоль, возникновение тромбоза все же чаще связано с тромбофилией, нежели со скрытой опухолью: 113 consecutive patients (50 males, 63 females) with first or recurrent VTE before the age 50, mean age (mean age 34.1 at time of VTE diagnosis) were followed retrospectively. Patients with known cancer were excluded for follow-up. The presence of known malignancy and concomitant cancer at the time of diagnosis of VTE and during follow-up (average 29 months) was investigated. The use of routine or extensive screening detection of occult cancer was also assessed. Extensive screening was performed in 61% of patients and compared with 39 % of routine screening. Overall of occult cancer was 1.8 %. The incidence of occult cancer was 1.2% during one year follow-up, and the prevalence at the time of diagnosis of VTE was 0.9%. The authors confirmed the low prevalence of occult cancer in cohort of consecutive young patients with VTE under the age of 50. It seems that only routine investigation with careful history taking, physical examination and simple laboratory tests are sufficient to detect of occult cancer in this age population. The authors recommend the follow-up of these patients, mainly those with idiopathic VTE. Inherited thrombophilia much more prevails in this young thrombophilia patients but risk of occult malignancy exists as well. --- Vnitr Lek. 2004 Oct;50(10):751-5.
