Как бы этот свет в конце туннеля не оказался фонарем поезда.. Мнение не ангиографиста.
Особенности биоабсорбируемых стентов: 1. После рассасывания исчезает тромбогенная/инородная металлическая поверхность. 2. Можно стентировать одну и ту же бляшку, пока ангиографисту не надоест. 3. Нет необходимости принимать плавикс+аспирин. 4. Если сиролимус, еверолимус или пакситаксель "сработают", то и рестенозов будет мало. Такой "средний" вариант получается между BMS и DES (мировая). 5. Растворимый стент будет решать проблему на время и исчезать, в случае возникновения проблемы повторно, можно будет сказат, что мы ни при чем. В случае тромбоза/рестеноза/проблемы можно сказать "не-не, мы ставили стент целыми 2 мм выше/ниже.. Ну мало ли, что вам кажеться, коллега, идите и дальше ломайте свои таблетки об колено". 6. Будет стоить дорого. 7. Процесс ТБКА стабильной бляшки подразумевает ее разрыв, как бы этот разрыв потом не аукнулся после рассасывания стента и смывания еверолимуса. 8. Юридически доказать факт установки стента после растворения будет трудно. 9. После последних событий с безопасностью DES, новый стент будут рассматривать "под микроскопом". Сравнение должно быть с DES, BMS и, возможно, медикаментозным лечением. |

Идея рассасывающегося стента слишком логична, чтобы работать. В кардиологии все "прорывные технологии" были нелогичны .
Обсуждается исследование на 30-ти больных с идеальными стенозами. В таком случае и баллонная ангиопластика без стента даст хороший результат.

Зилт - Тромбоз ?
За последние два месяца столкнулись с 2 случаями подострого тромбоза "Сypher" у пациентов которые принимали Зилт, есть ещё один пациент с рецидивирующими тромбозами "Cypher" на Зилте, однако в данном случае была выявлена патология свёртывающейся системы.
Нет ли у кого нибудь подобных наблюдений? |

The CoStar is a drug-eluting stent (DES) that has laser-cut reservoirs containing bioresorbable polymer loaded to elute 10 µg paclitaxel over 30 days. Methods: The authors randomized patients undergoing percutaneous coronary intervention (PCI) for a single lesion per vessel in up to three native epicardial vessels in a 3:2 fashion to CoStar or the Taxus paclitaxel-eluting stent. The primary endpoint of the study was 8-month major adverse cardiac events (MACE), defined as death, myocardial infarction (MI), or clinically driven target vessel revascularization (TVR). Protocol-specified 9-month angiographic follow-up was performed in 286 patients (457 vessels). Results: Of the 1,675 evaluable patients, CoStar was used in 989 and the Taxus stent was used in 686. The majority (1,330; 79%) of the patients underwent PCI for single-vessel disease, whereas 345 (21%) underwent multivessel PCI. At 8 months, there was no difference in death or MI, whereas the composite MACE rate was 11.0% for CoStar versus 6.9% for Taxus (p < 0.005). Clinically driven TVR (8.1% vs. 4.3%) was more common with CoStar. In-segment late loss was 0.49 mm with CoStar and 0.18 mm with Taxus (p < 0.0001). Imputation analysis suggested that CoStar had an efficacy similar to placebo. Conclusions: The CoStar DES is inferior to the Taxus DES with respect to its antirestenotic efficacy. |

PCI vs CABG for ULMCA

[Ссылки доступны только зарегистрированным пользователям ] BACKGROUND Several studies have compared the treatment effects of coronary stenting and coronary-artery bypass grafting (CABG). However, there are limited data regarding the long-term outcomes of these two interventions for patients with unprotected left main coronary artery disease. METHODS We evaluated 1102 patients with unprotected left main coronary artery disease who underwent stent implantation and 1138 patients who underwent CABG in Korea between January 2000 and June 2006. We compared adverse outcomes (death; a composite outcome of death, Q-wave myocardial infarction, or stroke; and target-vessel revascularization) with the use of propensity-score matching in the overall cohort and in separate subgroups according to type of stent. RESULTS In the overall matched cohort, there was no significant difference between the stenting and CABG groups in the risk of death (hazard ratio for the stenting group, 1.18; 95% confidence interval [CI], 0.77 to 1.80) or the risk of the composite outcome (hazard ratio for the stenting group, 1.10; 95% CI, 0.75 to 1.62). The rates of target-vessel revascularization were significantly higher in the group that received stents than in the group that underwent CABG (hazard ratio, 4.76; 95% CI, 2.80 to 8.11). Comparisons of the group that received bare-metal stents with the group that underwent CABG and of the group that received drug-eluting stents with the group that underwent CABG produced similar results, although there was a trend toward higher rates of death and the composite end point in the group that received drug-eluting stents. CONCLUSIONS In a cohort of patients with unprotected left main coronary artery disease, we found no significant difference in rates of death or of the composite end point of death, Q-wave myocardial infarction, or stroke between patients receiving stents and those undergoing CABG. However, stenting, even with drug-eluting stents, was associated with higher rates of target-vessel revascularization than was CABG. |
