The aim of the study was to determine those cut-off values of OGTT at gestational weeks ≤16, which can predict or exclude subsequent onset of GDM in a high risk group. Methods: Pregnant women at high risk of gestational diabetes (n = 163) underwent a 2-h, 75-g oral glucose tolerance test at gestational weeks ≤16 were analyzed in this study. In the event of a negative result, subsequent oral glucose tolerance tests were performed at gestational weeks 24–28 and 32–34. The sensitivity, the specificity, the positive and negative predictive values and the Odds ratio of the best cut-off values of fasting and postload glucose levels were calculated. Results: The best cut-off values to exclude subsequent GDM for fasting and postload glucose were 5.0 and 6.2 mmol/l, respectively. In combination, the best cut-off values were 5.3 mmol/l for fasting and 6.8 mmol/l for postload glucose, with negative predictive values of 0.97 and 0.71 and sensitivities of 96.9 and 86.3 at gestational weeks 24–28 and 32–34, respectively. Combination of these cut-off values with obesity proved to be very predictive for gestational diabetes by gestational weeks 32–34, with an Odds ratio of 6.0 [95% confidence interval: 1.7–21.0]. Conclusions: With regard to the very high negative predictive value of the method, pregnant women with glucose levels of ≤5.3 mmol/l at fasting and of ≤6.8 mmol/l at postload in gestational weeks ≤16 should undergo subsequent oral glucose tolerance testing merely at gestational weeks 32–34. Approximately a quarter (24.5%) of the pregnant women at risk of gestational diabetes satisfied these criteria. На самом деле даже американцы НЕ МОГУТ провоести ОГТТ всем беременным- хотя там это полагается сделать. Своими ушами слышала , как кто-то из суперважных гинекологов США объяснял ситуацию тють в тють как я это делаю на занятиях- давайте вначале определим. кому и КАКОЙ тест мы обязаны сделать. В целом в стране назрела и даже перезрела необходимость больших работ междисциплинарных на эту тему( надеюсь, что получится)

Статья о целесообразности проведения тотального GTT беременным.
У американцев есть растворы глюкозы 100 г и 50 г с апельсиновым вкусом. Я даже выпил как-то пол бутылочки. Делали в 2000 всем подряд в 22-24 недели и повторяли у беременных высокого риска в Нью-Йорке. Group A women (n=1338) were universally screened for gestational diabetes mellitus (GDM) and GDM patients were intensively treated. In Group B (n=4035), screening was performed only in women at high risk for GDM and treatment was conventional. This study confirms the validity of a cost-effective screening program for the diagnosis of GDM and that selective screening may be an option only in a situation where healthcare resources are very scarce and/or universal screening of any kind is not feasible. Once the diagnosis of GDM has been made, metabolic management with an intensive approach is important to reduce maternal and fetal morbidity. Diagnosis of GDM and intensive treatment represent a cost for the public health system, but permit a significant monetary savings in terms of costs linked to maternal and neonatal morbidity. |

Да, я уже говорила в одной из дискуссий ( в той дискуссии ратовали за ОГТТ всем - всем- всем - т.е и мужчинам с брюшком, и пухленьким. и родственникам), что даже сама навеска глюкозы в 75 гр - это не так просто и обсуждается , с какой глюкозой ( следуют биохимические тонкости) как сделать..
Но если с кото и начинать - так это с осознания проблемы- нужно или не нужно нам проводить ОГТТ, раз уж мы метформином вот у этой с инсулинорезистентностью беременность вызвали...М.б, и потери будут меньше плодов ( тема уже пошла- т.е тема уже дана). В отличие от американцев канидские собратья ( там на треть наш с Вами народ) - считает ОГТТ у беременных зpяшным делом- хоть наши и "таки гарни, таки повнi" . Я почти убеждена, что вот здесь кроме нас самих никто не решит, надо или нет нам прроводит беременным из групп риска ОГТТ- так и быть, первородящим тростинкам без родственников с диабетом моложе 25 лет не будем - а об остальных пора думать..

Уважаемая Галина Афанасьевна,
скажите, пожалуйста, как Вы относитесь к определению гликозилированого гемоглобина? У нас по-прежнему его назначают, но я читал, что этот тест устарел и не указывает четко на степень колебаний уровня глюкозы (на адекватность инсулинотерапии)... |

Гликогемоглобин - один из важнейших показателей адекватности контроля диабета, позволяющий оценить долгосрочную ( 3-х месячную ) гликемию.Гликогемоглобин не заменяет и не подменяет самоконтроль и определение гликемии, но в совокупности позволяет лучше охарактеризовать риски осложнений.
Гликогемоглобин указывается ка кодна из основных целей лечнеия диабета- а проблемы его определения есть во всех лабораторных показателях.. Пожалуйста, путь пациент ставит систему мониторирования глюкозы,- - и измеряет глюкозу каждые 15 мин...

При беременности определение гликогемоглобина не будет информативным. Показатель-то интегральный за 3 месяца. А углеводный обмен при беременности может "сломаться" в более короткие сроки.
