Central venous retinopathy was reported in two young women after oral treatment with tranexamic acid for menorrhagia (33).
A 56-year-old man, who had been undergoing hemodialysis for 1 year after 10 years of peritoneal dialysis, was given intravenous tranexamic acid after an emergency operation for bleeding peptic ulcers; he had gradual loss of sight over about 1 week, and 2 weeks after the operation became blind (34). Retinography was flat and his visual field was narrowed. Fluorescein angiography showed microgranular hyperfluorescence, suggesting malfunction of the pigmented layer of the retina. There were no abnormalities in the brain or optic nerve on MRI scanning. Withdrawal of tranexamic acid restored his sight within a few days. He had received tranexamic acid once before because of a bleeding ulcer, and his sight had been impaired at that time too.
However, in 14 patients who had taken tranexamic acid for an average period of 6 years there was no evidence of ocular damage (35).
