На этот вопрос я могу ответить. Обычно, с точки зрения влияния на гемодинамику, значимой является бляшка более 50% по NASCET или более 75% по ECST. Однако, не почти в половине случаев правильно измерить бляшку по УЗИ планиметрически невозможно. Это может происходить из-за выраженного кальциноза, из-за сложной пространственной геометрии или из-за неудовлетворительной визуализации границ бляшки и проходимой части сосуда. Кроме того, в нашей стране врачи УЗД очень редко указывают каким методом измеряли стеноз.

Мы использовали следующие критерии для увеличения точности дуплекса сосудов шеи: peak systolic velocity (PSV) >230 cm/s, PSV (common carotid artery)/PSV (ICA) >4, peak end-diastolic velocities >100 cm/s, and North American Symptomatic Carotid Endarterectomy Trial (NASCET) criteria for determining the degree of stenosis. Carotid duplex imaging was not used in patients with severe plaque calcification/poor imaging, the degree of stenosis in these patients was determined only by CT/MRI. We used the following ultrasound classification scheme to classify stenosis: <50% stenosis when ICA PSV was <125 cm/s and plaque or intimal thickening was visible; 50% to 69% stenosis when ICA PSV was 125 to 230 cm/s and plaque was visible; >70% stenosis to near occlusion when ICA PSV was >230 cm/s and visible plaque and lumen narrowing were seen; near occlusion when there was a markedly narrowed lumen at color Doppler; and total occlusion when there was no detectable patent lumen at grayscale ultrasound and no flow at spectral, power, and color Doppler ultrasound imaging.
