Abstract A search of the literature revealed no documentation of the complication rate associated with percutaneous catheterization of aorto-femoral bypass grafts in the arteriographic evaluation of graft patency. The authors compared the complication rates of 29 consecutive arteriograms performed through grafts (Group I) with 40 arteriograms performed on non patients (Group II). Abstract To assess the safety of percutaneous puncture for the angiographic study of aortofemoral bypass grafts, we reviewed the medical records of 58 patients who underwent a total of 86 graft punctures. Short- and long-term follow-up revealed that in 83 punctures there were no complications. In three instances (3.5%) there was acute thrombosis of the punctured limb of the graft. This complication may have expedited but did not substantially alter the surgical plans of managing the underlying clinical problem. Although the incidence of complications of direct graft puncture is higher than that associated with puncture of native vessels, alternative approaches are also associated with complications, many of which may be more serious. Therefore, for angiographic studies of patients with aortofemoral bypass grafts, we favor the direct percutaneous puncture provided that enough time has elapsed from surgery. До недавнего времени проходили пациенты с графтами, нуждаюшиеся в химоемболизации - суперселективная катетеризация ветвей печёночной артерии через графт сложна, да и при селективной почечной приходилось попотеть....

То что катетер в узел можно связать - это я знаю. Знаю еще что когда-то интродьюсеры считали за ненужное баловство. В общем, отращиваю усы и мотаю, мотаю..... Thanks a lot!!! |

Заметно снижается торк; надо титрoвать скорость манипуляции, чтобы не повредить стенку катетера...

При введении катетера через аорто-феморальный графт для хемоемболизации (ситуация относительно редкая) стараются использовать 4F без интродьюсера.
При манипуляции есть шанс стенку повредить и даже минимальная возможность "утечки" емболизата в дистальное русло должна быть предотврашена лимитацией врашений.... |

Из японского опыта следует следуюшее: "..Catheter Implantation Celiac angiography was performed according to the femoral approach under local anesthesia. After the detection of HCC and its feeding arteries, a 5-Fr heparin-coated catheter was introduced into the proper or common hepatic artery. The gastroduodenal artery and the right gastric artery were occluded with metallic coils to prevent gastroduodenal injury by the anti-cancer drugs. Aberrant hepatic arteries, if present, were occluded with a metallic coil or a mixture (1:1.5) of n-butyl cyanoacrylate and iodized oil prior to the treatment for hepatic arterial redistribution (30). After the catheter was connected to the injection port, the device was implanted in a subcutaneous pocket in the femoral site. No patients received anticoagulant therapy. The device was filled with 5000 IU of a heparin solution to prevent occlusion. Before treatment, vascularity of tumors and enhanced area by contrast medium, were confirmed using CT angiography through an implanted reservoir catheter in all patients..."
