трудно не согласиться с тем, что последние рекомендации уравнивают диуретики с иапф, кальциевыми блокерами и т.д.
но, на сколько мне не изменяет мой склероз, не изменилось мнение о резистентной АГ, относительно которой сказано, что ето такая штука которая плохо лечится тремя препаратами один из которых диуретик. потому мне кажется вполне себе допустимой мысль, что диуретик ето важно p.s. тиазидные диуретики люблю, потому в данном случае солидарен с англичанами |

Британия, как известно, - родина бета-блокаторов и страна с самым большим опытом критическо-аналитического отношения к оным (несмотря на Нобелевскую премию Sir James Black). Во многом, позиции США и Британии (плюс Канады, Австралии и Новой Зеландии сходны). Вы цитируете "европейские" рекомендации 2007 г., мои же ссылки на 2008-2011. Но я не догматик. Лишь бы помагало. |

Классическая дискуссия никогда не бывает лишней А вот господин Осипов говорит, что все равно, какие препараты назначать, лишь бы давление снижали Ну и чтобы, конечно, это были препараты с положительной доказательной базой, а не доксазозин или клофелин.

Beta-blockers can precipitate bronchospasm and should therefore usually be avoided in patients with a history of asthma. When there is no suitable alternative, it may be necessary for a patient with well-controlled asthma, or chronic obstructive pulmonary disease (without significant reversible airways obstruction), to receive treatment with a beta-blocker for a co-existing condition (e.g. heart failure or following myocardial infarction). In this situation, a cardioselective beta-blocker should be selected and initiated at a low dose by a specialist; the patient should be closely monitored for adverse effects. Atenolol, bisoprolol, metoprolol, nebivolol, and (to a lesser extent) acebutolol, have less effect on the beta2 (bronchial) receptors and are, therefore, relatively cardioselective, but they are not cardiospecific. They have a lesser effect on airways resistance but are not free of this side-effect.
Contra-indications asthma (but see notes above), uncontrolled heart failure, Prinzmetal’s angina, marked bradycardia, hypotension, sick sinus syndrome, second- or third- degree AV block, cardiogenic shock, metabolic acidosis, severe peripheral arterial disease; phaeochromocytoma (apart from specific use with alpha-blockers, see also notes above) Bronchospasm Beta-blockers, including those considered to be cardioselective, should usually be avoided in patients with a history of asthma or bronchospasm. However, when there is no alternative, a cardioselective beta-blocker can be given to these patients with caution and under specialist supervision. |

Уважаемый Евгений, я ссылаюсь на Reappraisal 2009, в которых есть отсыл к рекомендациям 2007 года, так как позиция в отношении классов препаратов не изменилась.
В отношении резистентной АГ, Ultimately, many patients will need administration of more than three drugs, и конечно, один из них, скорее всего будет диуретиком. Но, мне кажется, мы забыли о первом шаге Цитата:

Calcium-channel blockers versus thiazide-type diuretics A meta-analysis of five studies (ALLHAT582-584, INSIGHT104,105, MIDAS89, NICS-EH341, VHAS510,649) comparing calcium-channel blockers with thiazide-type diuretics found no significant differences for mortality, MI or stroke. There was a statistically significantly higher incidence of heart failure with CCBs (RR 1.38, 95% CI 1.25 to 1.53). |

Проблема "континента", ИМХО, заключется в неоднородности его подхода к решению самых разнообразных проблем (включая артериальную гипертонию): Thiazide diuretics in hypertension An article from the E-Journal of the ESC Council for Cardiology Practice Vol 8 N°35 2 June 2010 The use of diuretics (and beta-blockers) to treat hypertension has known a dramatic decline since the early 1980’s through to the early 1990s with the introduction of ACE-inhibitors and calcium channel blockers. However, thiazide diuretics are still a first line option to treat arterial hypertension, due to their efficacy and cost-effectiveness profile. We have presented major trials on this subject in this review. Background Thiazide Diuretics have been used since the late fifties in the treatment of hypertension and remain one of the most important group of drugs used to reduce blood pressure, due to their efficacy and cost-effectiveness profile. The use of diuretics (and beta-blockers) to treat hypertension has known a dramatic decline since the early 1980’s through to the early 1990s with the introduction of ACE-inhibitors and calcium channel blockers. Thiazides were used in much higher doses then and concerns were raised regarding their metabolic effects (1). Из моего личного опыта на диуретиках высокий комплайенс, особенно по сравнению с сартанами и антагонистами кальциевых каналов. Сартан стоит дорого, поэтому приём часть больных самостоятельно прекращают. Антагонисты кальциевых каналов кроме того, что являются самыми дорогостоящими, так ещё и на последнем месте по переносимости.
Ингбиторы АПФ особенно у пожилых уступают остальным классам. Иммеется ввиду влияние на конкретные цифры АД. Индапамид за 3 доллара лично у меня в первых рядах, при цифрах давления 180 и выше. |
