Статьи, исследования конечно должно и нужно изучать всем ЛОРам, но реальная жизнь зачастую вносит такие коррективы, которые не соответствуют никаким даже самым достоверным исследованиям.Вы вероятно видели не так много синуситов, а у меня каждую неделю больные дети получавшие амбулаторное, БЕЗПУНКЦИОННОЕ лечение с 2мя и даже 3мя курсами а/б-терапии в течении нескольких недель(зачастую попадавшими в спектр а/б-ми!!!) для которых подобное лечение закончилось осложнениями и как результат гаймороэтмоидотомией или в лучшем случае хронизацией процесса. А сравнение с аппендиксом вполне достоверное, потому что и то, и другое при консервативном подходе "русская рулетка" и не так уж и важно где гной, в животе или голове.

В диагностической пункции и по американским рекомендациям нуждается только часть заболевших:
If patients do not improve while receiving the usual dose of amoxicillin (45 mg/kg/d), have recently been treated with an antimicrobial, have an illness that is moderate or more severe, or attend day care, therapy should be initiated with high-dose amoxicillin-clavulanate (80-90 mg/kg/d of amoxicillin component, with 6.4 mg/kg/d of clavulanate in 2 divided doses). This dose of amoxicillin will yield sinus fluid levels that exceed the minimum inhibitory concentration of all S pneumoniae that are intermediate in resistance to penicillin and most, but not all, highly resistant S pneumoniae. There is sufficient potassium clavulanate to inhibit all b-lactamase producing H influenzae and M catarrhalis. Alternative therapies include cefdinir, cefuroxime, or cefpodoxime. A single dose of ceftriaxone (at 50 mg/kg/d), given either intravenously or intramuscularly, can be used in children with vomiting that precludes administration of oral antibiotics. Twenty-four hours later, when the child is clinically improved, an oral antibiotic is substituted to complete the therapy. Although trimethoprim-sulfamethoxazole and erythromycin-sulfisoxazole have traditionally been useful in the past as first- and second-line therapy for patients with acute bacterial sinusitis, recent pneumococcal surveillance studies indicate that resistance to these 2 combination agents is substantial.51,52 Therefore, when patients fail to improve while receiving amoxicillin, neither trimethoprim-sulfamethoxazole nor erythromycin-sulfisoxazole are appropriate choices for antimicrobial therapy. For patients who do not improve with a second course of antibiotics or who are acutely ill, there are 2 options. It is appropriate to consult an otolaryngologist for consideration of maxillary sinus aspiration to obtain a sample of sinus secretions for culture and sensitivity so that therapy can be adjusted precisely. Alternatively, the physician may prescribe intravenous cefotaxime or ceftriaxone (either in hospital or at home) and refer to an otolaryngologist only if the patient does not improve on intravenous antibiotics. Some authorities recommend performing cultures of the middle meatus instead of aspiration of the maxillary sinus to determine the cause of acute bacterial sinusitis.53 However, there are no data in children that have correlated cultures of the middle meatus with cultures of the maxillary sinus aspirate. Все, сдаюсь. В России другие синуситы: Sinusitis soveticus. В Америке - Sinusitis americanus. Поэтому и лечение разное.

3-4 года назад я работал в крупной(для нашего города) частной клинике, довольно часто ко мне обращались русскоязычные пациенты проживающие зарубежем и приехавшие в Россию к родственникам или по делам. Чаще из Англии, Германии, иногда из восточной Европы, было 2 человека из Австралии и у всех практически одинаковый анамнез: клиника о.синусита-в назначениях ГКС спрей, ни дополнительных исследований, ни местных процедур-результат у некоторых, переход в гнойную форму; была пациентка с субатрофическим ринитом-опять же ГКС, назначенный длительным курсом-результат носовые кровотечения, изъязвление слизистой и т.д.

Patients with clear anatomic abnormalities or sinonasal polyps may be more likely to respond to surgical therapy. Functional Endoscopic Sinus Surgery Indications Functional endoscopic sinus surgery is based on several key observations: (1) widely patent antrostomies in nonanatomic positions may fail to drain sinuses due to the directionality of mucociliary flow; (2) the osteomeatal unit is anatomically constricted; and (3) the stripping of sinus mucosa leads to delayed healing and the loss of normal ciliary function. Thus, a conservative endoscopic technique has been developed. The keys to the technique are the use of "through-cutting" instruments that preserve sinonasal mucosa and the excellent visualization made possible with modern telescopes. Mucosal polyps can be carefully debrided, the natural ostia enlarged, and the ethmoid sinuses unroofed, which opens them to the nasal cavity. The improvement in symptoms with functional endoscopic sinus surgery may be expected in more than 90% of patients. Open Sinus Surgery In spite of the versatility of endoscopic procedures, open sinus surgery is sometimes needed. One example is the Caldwell-Luc antrostomy in which the maxillary sinus is entered through a sublabial incision. The Caldwell-Luc approach allows biopsy of the sinus contents; also, once the sinus is entered, a drainage window may be made into the nasal cavity." ? P.S. Что же такое "местные процедуры"? |
