But for those with no reduction in volume, there is a proarrhythmic effect.":ai:

"They indicate that CRT-D reduces ventricular arrhythmia risk better than ICD therapy alone in mild heart failure only if it successfully promotes reverse remodeling, she notes, and "biventricular pacing with epicardial left ventricular pacing, without improvement in ventricular size and function, appears to be proarrhythmic. It is thus incumbent upon us to select patients for CRT carefully who have the most likelihood of responding to resynchronization and to make every effort to place the [LV pacing] lead optimally."

"Even the patients who were high responders by echocardiography still had ventricular arrhythmia events, although their risk was very low," he said. For those without such good CRT responses at one year, the analysis suggests they should have intensified care. "You should look after them much more strictly, maybe give them more antiarrhythmic medications, because those patients are at very high risk. The ICD will discharge a lot."
